“Evidence-based practice (EBP) involves clinical reasoning in three major domains: best-available scientific evidence, clinical judgment (experience), and patient’s perspective (Figure 1). Evidenced-based wound care seeks to integrate clinical wisdom with the best available science to optimize patient care with safety, statistical power and efficacy brought to bear on the problem at hand, the wound(s).1 This paper provides perspective primarily in the domain of best-available scientific evidence as it pertains to whirlpool (WP) use in wound care. Whirlpool, one of the oldest types of hydrotherapy, was originally used by physical Screening Library therapists (PTs) to treat patients with burns in need of extensive debridement. In many areas of the United States, WP remains an active component of wound care as a means for the removal of necrotic cellular debris and contamination. With the advent of other options, using water as a cleansing agent, it is important to critically analyze the literature reporting the effects of WP.
